Every coach in the Fargo Soccer Club has to complete the following tasks in order to coach a competitive team.
1. Complete and return the Coach Code of Conduct. Download the form here. You can email a PDF version of the completed form to

2. Complete the US Club Soccer Background Check form here. You can skip over the information at the previous link and go right to the background check here.
3. Email
an electronic photo of yourself in JPG format. Try to make it a high resolution photo of just your head and shoulders.
4. Complete the concussion management form by visiting http://www.cdc.gov/concussion/HeadsUp/Training/HeadsUpConcussion.html. You must take this course. When you are finished, please print a copy of your certificate and mail it to me. You can print it to a PDF file and email it to me at
